A&M Care, LLC (“A&M Care,” “we,” or “us”) provides attendant care, habilitation, respite, and homemaker services in Maricopa County, Arizona. This policy explains what information we collect through amcarearizona.com, how we use it, and the choices you have.
1. What this policy covers
This policy covers our website and the ways you contact us through it, including email and our online job application.
It does not cover the health and care information we keep about the people we serve. Federal privacy law (HIPAA) and Arizona rules protect that information, and our separate Notice of Privacy Practices explains how we handle it. We give that notice to every member and family when services begin, and you can read it here or ask us for a copy at any time.
2. Please don’t send health information by email
Email isn’t a secure way to share medical or personal details. If you’re contacting us about services for a family member, send your name and the best way to reach you, and we’ll follow up by phone. Please don’t email diagnoses, AHCCCS or Social Security numbers, or medical records.
3. Information we collect
When you email us. We receive your email address, your name if you include it, and whatever you write.
When you apply for a job. Our online application is hosted by Fillout, a separate company that runs the form for us. This may be a short interest form followed by a full application. What you enter goes to that company’s secure systems, not to our website. It can include your contact details, work history, answers about eligibility (such as your age, Arizona residence, and fingerprint clearance), and documents you upload, like a copy of your clearance card. Section 5 explains what happens next.
Information about other people. If you list references, or tell us about a family member or friend who may want to work with us, we’ll use their contact details only to reach them for that purpose. Please let them know first.
When you visit the site. Our hosting provider, Cloudflare, automatically records basic technical details, such as your IP address, browser type, and the pages you view. This keeps the site running and protects it from abuse. The site also loads fonts from Google Fonts, and those services can see your IP address when your browser requests them.
We don’t use advertising cookies, tracking pixels, or analytics tools. If that changes, we’ll update this policy before we start.
4. How we use information

- To review job applications and hire caregivers
- To meet the requirements of the Arizona Division of Developmental Disabilities (DDD), AHCCCS, and other agencies that oversee our work
- To keep our website working and secure
We don’t sell or rent personal information, and we don’t use it for advertising.
5. Job applicants
Where your application goes. Your application is stored with our form vendor, and only the A&M Care staff who handle hiring can see it. We also keep a secure archive copy in A&M Care’s business account with Google Workspace so we can meet Arizona’s record rules. If we hire you, your information moves into our payroll system (Paychex) and our employee records system (VTrack).
Background checks. Arizona requires caregivers to hold a valid fingerprint clearance card and pass background checks. We’ll ask for your written permission before running any check. That step happens separately from the application.
References. We may contact the references you list.
How long we keep applications. Arizona requires us to keep every job application and its related records for at least five years, including applications we don’t move forward with. After that, we delete them securely.
Equal opportunity. A&M Care is an equal opportunity employer. Any questions about veteran or disability status are optional. We store those answers separately and never use them in hiring decisions.
6. Who we share information with
- Companies that work for us, such as our website host, email provider (Google Workspace), form vendor, and payroll provider. They may use your information only to do that work for us.
- Agencies and health plans that oversee our services, such as DDD and AHCCCS, when the law or our contracts require it.
- Others when the law requires it, such as in response to a court order, or to protect someone’s safety.
- A new owner or a new name. If A&M Care changes ownership or its name, your information may move to the new business. This policy will keep applying to it.
7. Children’s privacy
Our website is meant for adults. We don’t knowingly collect information from children under 13 through it. Many of the people we serve are children, and their information is protected under our Notice of Privacy Practices, not this policy. If you believe a child has sent us information through the website, contact us and we’ll delete it.
8. Text messages
[Keep only if A&M texts applicants or staff] If you give us your mobile number and check the box agreeing to receive texts, we may text you about your application, your schedule, or your work with us. How often we text varies. Message and data rates may apply. Reply STOP at any time to stop receiving texts, or HELP for help. We never share your mobile number or your consent to receive texts with anyone for their marketing.
9. How we protect information
Our website uses encrypted connections (HTTPS), and we don’t store applications or sensitive personal information on the website itself. Only the people who need it for their work can see your information. No system is perfectly secure. If a breach affects your personal information, we’ll notify you as Arizona law requires.
10. How long we keep information
- Emails: as long as we need them to respond and for our business records, up to [period].
- Job applications: at least five years, as described in section 5.
- Website technical records: kept by our hosting provider for a short period under its own policies.
11. Your choices
You can ask to see, correct, or delete the personal information we hold about you. Email us and we’ll respond within 30 days. We may have to keep some records when the law requires it, such as job application records. You can stop text messages at any time by replying STOP.
12. Changes to this policy
If we change this policy, we’ll post the new version on this page and update the date at the top.
